The nurse provides an advanced level of nursing care across the health care continuum to meet the specific physiological and psychological needs of patients with acute, critical and/or complex chronic health conditions.
Clinical Responsibilities and Key Functions:
- Performs initial patient screening, patient history and physical examinations to determine treatment plan and/or manage medical condition.
- Updates patient health information and ensures accuracy of information at each patient visit, ensuring continuity of care and the ability to collaborate with other health professionals in the treatment of each patient. Analyze test results and imaging reports to determine the need for follow-up time and/or additional treatments.
- Serves as the resource patients need for ongoing care information, counseling and guidance for all critically ill patients.
- Performs chronic health examinations to determine health status and develop a treatment plan, including ordering labs, imaging, referrals and/or prescribing medications.
- Performs differential diagnoses, which includes ordering, performing, and interpreting diagnostic laboratory tests, including radiologic tests and EKGs.
- Prescribe pharmacological and non-pharmacological treatment methods in the direct treatment of acute and chronic diseases.
- Respond to on-site emergencies until emergency services arrive. Uses medical training and knowledge to respond to patients' rapidly changing physiological status.
- Assesses response to therapeutic interventions and recommends continuation/modification of treatment plan.
- Directs and clinically directs the work of nurses and other members of the care team.
- Acts in accordance with legal and ethical professional obligations (eg, patient management, documentation, advance directives).
- Required to comply with all state laws and required to report. Performs roles and duties in accordance with State Board of Nursing and Evernorth Care Group policies.
